Dr. Clayton Lewis and Dr. Miranda Fisk are both certified medical examiners listed on the National Registry of Certified Medical Examiners, allowing drivers to complete DOT physicals locally while meeting national certification requirements.
Appointments are available for individuals and groups, and we can accommodate large groups as needed.
Who Needs a DOT Physical?
DOT physicals are required for:
- Commercial drivers obtaining or renewing a CDL
- Drivers required to maintain a valid Medical Examiner’s Certificate
- Employers seeking DOT-compliant exams for their drivers
What does the DOT Physical Exam include?
- Review of health history
- Blood pressure assessment
- Vision screening
- Hearing evaluation
- Review of systems
- Urinalysis (blood, sugar, protein, and specific gravity)
Cost and Insurance
- DOT Physical Exam Fee: $85
- Insurance is not accepted for DOT physical exams
- Payment is due at the time of service
What to Bring to Your Appointment
Please bring the following items to your DOT physical exam:
- A valid driver’s license
- Your current Medical Examiner’s Certificate (if applicable)
- Eyeglasses or contact lenses (if required for driving)
- A list of current medications, including dosage
If you are insulin-dependent diabetic or use a CPAP machine, additional documentation is required for your visit. Please call our office in advance so we can review what is needed.
For all other medical conditions, documentation requirements can be discussed by phone prior to your appointment to help reduce delays in certification.
